Normally we cut our nails straight and if we cut in rounding fashion then there is a high chance of ingrowing toe nail which grows in the surrounding soft tissue causing inflammation and infection.

Due to chronic diabetes mellitus patient develops loss of sensation and decrease in blood supply to feet which in turn becomes prone to injury secondary to trauma, burns etc.

Soft tissue involves skin, subcutaneous tissue, muscles, nerves and arteries
So any tissue involved in injury should be replaced with like tissue.

Flap is a soft tissue used to cover the defect while maintaining its blood supply. Depending on the donor site availability and choice of surgeon it can be local flap, regional flap or free flap.

This is a pigmented skin cancerous lesion rarely found in our population.
